Youth social work support services

Connect Health & Community’s youth social work service aims to keep young people well connected to their family and community, and ensure they have safe and stable housing.

How can the youth social work service help me?

If you’re aged between 12 and 25, Connect Health & Community can help you and your family (if appropriate) through:

  • one-on-one counselling
  • individual casework support
  • referring you to other services
  • health and wellbeing projects and activities, some of which are run in partnership with local schools and other organisations.

Am I eligible?

You may be eligible to access our youth social work service if you are: 

  • between 12 and 25 years old
  • living, working and/or attending school or college in the Bayside, Glen Eira or Kingston areas.

If you’re not eligible, don’t worry – we can help your find the support you need, either with us or with another service.

Where do I need to go to get help?

You can either make an appointment to come in to Connect Health & Community’s offices in Bentleigh East or Cheltenham, or we can organise to meet you at school, at home, or anywhere you feel comfortable.

Launch Housing can provide assistance with crisis accommodation as well as medium to long term housing support, including potential financial assistance. They have branches in Cheltenham and in St Kilda.

headspace is a national youth mental health foundation. Drop in to one of their centres or go online for free information and support.

Connect Health & Community’s youth social work service is available on weekdays during business hours. It is a funded program of Department of Health and Human Services under the ISHY model.
