What is My Aged Care?

My Aged Care is the Commonwealth one-stop-shop for aged services and information in Australia. The services include allied health services, aged care homes, respite and help in your own home including community transport.

It is:

  • for eligible people aged 65 years and over
  • for Aboriginal and Torres Strait Islander people, people on a low income who are homeless or at risk of homelessness, 50 years and over
  • and for their carers.

There is a registration process with My Aged Care. Please visit this page for further information.
When you contact the My Aged Care Centre they will register you and conduct a screening process over the phone and then either:

  • refer you directly to a  service provider such as Connect Health & Community
  • refer you for a face to face assessment conducted by the My Aged Care Regional Assessment Service
  • provide information about non-Commonwealth funded services as appropriate.

Once you have been referred to Connect Health & Community by the My Aged Care service our staff will contact you. Connect Health & Community is a provider of the Commonwealth Home Support Program (CHSP) and of the Home and Community Care (HACC) program, which provides services to eligible people.
