Promoting health and wellbeing

Community health nurses help with general health support at home. We work as part of a team to help you understand and manage your health care, and provide links to other services if you need them.

The services can be provided at Connect Health & Community, and/or in the home. We might also suggest activities which are provided at other locations. Your community health nurse will work with you, your family if required, your GP and other practitioners to provide a health plan for you.

How can community health nurses help?

We provide a range of medical and nursing advice to support people experiencing:

  • Aged care issues
  • Diabetes
  • Cardiac rehabilitation
  • Chronic health conditions
  • Social isolation

We also work to support:

  • Carers in the community
  • Young people’s health
  • Family relationships
  • Parent & child attachment

Who is eligible?

Everyone in the community is welcome to access our community health nursing service.

Do I need a referral?

No. You can access our community health nursing service by getting in touch with us and requesting an appointment.

What about My Aged Care?

You may be referred to community health nursing as part of a My Aged Care assessment. This referral will be discussed with you and will be made with your permission.
