Medicare Funded Allied Health

Connect Health & Community has the following Medicare-funded services available at our Bay Road Cheltenham site.

Physiotherapy, Podiatry, Exercise Physiology, Diabetes Education and Psychology.

Connect Health & Community will participate in Team Care Arrangements (TCA) as part of the Chronic Disease Management program managed by a General Practitioner.

If the patient is currently being actively treated at Connect Health & Community, then your request to join the TCA will be forwarded to the relevant practitioner for signing and return. Address or fax the documents to Connect Health & Community Intake.

If your patient is not being actively treated then they will require a referral to the designated service. The TCA cannot be signed until the client has been registered and been seen or contacted by the relevant practitioner.

Connect Health & Community Intake operates 9.30am to 4.30pm Monday to Friday, except for public holidays.

Connect Health & Community prefers the General Practice Referral from GP practices to enhance patient referrals.

As Connect Health & Community usually has to contact the client by telephone, correct phone contact numbers for patients or carers assist in faster service.
