NDIS Access Support

We understand that trying to access the NDIS can be difficult at times.

And we know that sometimes it can be hard to understand what information you are being asked to provide, let alone that simply filling out the forms can be complicated.

But the NDIS is an important service to help you and your family get the best quality of life, so we are here to help.

Our NDIS Access Support can help you, if you:

• Need support but have not applied for the NDIS because you don’t know where to start.
• Have started the NDIS application process, but found it too difficult and stopped part way through.
• Are currently in the process of applying for the NDIS and could use some extra support.

What help is available?

Connect Health & Community can help you:

• Complete NDIS application forms;
• Undertake the allied health assessments (for example, OT or psychology) you need to provide evidence with your application;
• Get advice and connections for assessments and reports from a doctor or specialist as part of your NDIS application process; and
• Get advice and connections to other services and supports if the NDIS is not right for you.

Am I eligible for support?

This services is for people living in Glen Eira and Bayside Council areas

Unfortunately we are unable to provide this service for children.

Substantially reduced capacity – to satisfy the NDIS access requirements, a person must demonstrate a substantially reduced capacity in one or more of the following areas: mobility; communication; socialising; learning; self-care; self-management.

If you between the ages of 18 to 64 years you are eligible for support to access the NDIS.
