How can diabetes education help me?
Our diabetes educator is a Registered Nurse with qualifications and expertise in diabetes education.

Who is eligible for diabetes education?
Adults with type 2 diabetes and adults with type 1 diabetes (if stable) are eligible. However, those with gestational diabetes or other diabetes-related to pregnancy are referred to other specialised services.
Do I need a referral?
Generally, yes. A referral from your GP and you will need a Chronic Disease Management Plan.
What about My Aged Care?
If you are 65 years or over, your referrer will need to contact My Aged Care to make a referral to a diabetes educator. If you are referring yourself, My Aged Care will assist you.
