Diabetes education

Connect Health & Community’s diabetes education service will help you learn about and manage your diabetes, and stay healthy.

How can diabetes education help me?
Our diabetes educator is a Registered Nurse with qualifications and expertise in diabetes education.

Working with a team of health professionals ‒ including dietitians, podiatrists and exercise physiologists ‒ Connect Health & Community’s diabetes educator can:

  • Help you learn about your diabetes and staying well.
  • Assist you to make healthy lifestyle choices
  • Help you with glucose monitoring and control.
  • Work with your GP, family or other support people.
  • Help you access information and resources, such as the National Diabetes Services Scheme (NDSS).
  • Link you to other services (diabetes centres at public hospitals, group education programs, counselling and dental services).

Who is eligible for diabetes education?
Adults with type 2 diabetes and adults with type 1 diabetes (if stable) are eligible. However, those with gestational diabetes or other diabetes related to pregnancy are referred to other specialised services.

Do I need a referral?
Generally, yes. A referral from your GP or healthcare provider is preferred. This can be discussed with our diabetes educator if required.

What about My Aged Care?
If you are 65 years or over, your referrer will need to contact My Aged Care to make a referral to a diabetes educator. If you are referring yourself, My Aged Care will assist you.

What are the fees for diabetes education?
Fees start at $10 for low income earners and are scaled upwards according to government guidelines. Call 03 9575 5333 to find out more.
