  • and for their carers.

For people under 65 years who have a disability and their carers please contact us directly for further information about alternative support on (03) 9575 5333.

There is a registration process with My Aged Care. Please visit this page for further information.
When you contact the My Aged Care Centre they will register you and conduct a screening process over the phone and then either:

  • refer you directly to a  service provider such as Connect Health & Community
  • refer you for a face to face assessment conducted by the My Aged Care Regional Assessment Service
  • provide information about non-Commonwealth funded services as appropriate.
